Rheumatoid Arthriris (RA) is a chronic autoimmune disorder that affects all joints intermittently, almost invariably one of the persistent symptoms, waxing and waning in intensity, and a progressive deterioration of joint structures leading to deformations and disability. Gradual progression to other organs such as lungs, heart etc are its complications.

Cause

Etiology is unknown. Genetic and environmental factors are the most common causes. There is strong involvement of immune system even in case of association with joints due to congestion.

Pathophysiology

Exact mechanism is not known. The auto-immune response produced results in the inflammation and thickening of joints affecting bone and cartilage. Three phases of progression of RA are an initiation phase (due to non-specific inflammation), an amplification phase (due to T cell activation), and chronic inflammatory phase (with tissue injury resulting from the cytokines and other factors).

 

Signs & Symptoms

RA has a wide range of symptoms typically being warm, swollen and painful joints. Pain and stiffness aggrevates during morning most commonly small joints are involved. Symptoms appear gradually over weeks to months. Other systemic diseases in association are Cardiovascular disease, Osteoporosis, Interstitial lung disease, Systemic lupus erythematosus, Psoriatic arthritis, Fibromyalgia, Psychological involvement etc.

Arthitis of joints:- involves inflammation of synovial membrane, swelling, tenderness, morning stiffness restricting movements. Usually starts in smaller joints of hands and foot, gradually progressing towards larger joints. Synovitis gradually gets attached to the adjacent tissue making a permanent deformity or loss of function of the affected joint. Ulnar deviationboutonniere deformity, swan neck deformity, Z-thumb deformity, hammer toe deformity are some of them.

Skin:- Rheumatoid nodule is the most common non-joint feature that is another inflammatory reaction which is a necrotizing granuloma. Erethyma nodusa, palmar erythema, diffuse alopecia areata etc are some other skin involvement.

Lungs:- Lung fibrosis, exudative pleural effusion, lung nodules are seen associated with RA.

Heart:- RA persons are more prone to Atherosclerosis, Myocardial infarction and Stroke.

Blood:- Anaemia due to chronic inflammation.

Other complications seen in RA are Renal amyloidosis, scleritis, autoimmune hepatitis, peripheral neuropathy, carpel tunnel syndrome

Constitutional symptoms including fatigue, low grade fevermalaisemorning stiffnessloss of appetite and loss of weight are common systemic manifestations seen in people with active RA.

 

Ayurveda Treatment For Rheumatoid

There is a successful history in the field of Ayurveda in managing Rheumatoid Arthritis, Fever etc. As though RA is a chronic disease the person’s quality of life can be improved which has been hampered by the disease. The main goal is to reduce pain, stiffness, inflammation and fever associated, if any.

Systematic treatment protocol to reduce Ama in first line of treatment relieves the progression of the disease. Internal Ayurveda medicine along with strict diet regime under an expert supervision will be followed. Good healthy anti-oxidant vegetables are selected according to the condition of the patient.

Panchakarma has a major role in pacifying pain, stiffness and inflammation. It involves a long procedure that take 21-30 days of different treatment procedures under strict diet regulations. The diet should be continued for another 30 days. Panchakarma follow-up treatment is advised for RA person yearly.

After purifactory treatment schedule the person will be advised a diet regime to be followed further to prevent the progression in future.

Yoga has a big role as it has many advantages over normal exercises. Especially in RA persons, it improves the blood circulation, enzymatic action, proper digestion, stimulates central nervous system and regulates hormonal secretion. Hence reduces inflammation all over the body. It reduces the progression of diseases. To reduce the risks for complications such as cardiovascular involvement, circulatory involvement etc, it maintains optimal control of inflammation caused by RA.
