Haemorrhoid, also known as Piles, is a chronic disease affecting the vascular structures in the anal canal. Haemorrhoids exhibits as a chronic constipation or painful bleeding while defaecation.

Cause

Exact cause of haemorrhoids is unknown. It has a remarkable cause of hereditary. Constipation, diarrheoa and sitting in toilet for long time are detected to be among common causative factor. It is also seen during pregnancy. Lack of exercise, prolonged straining of defeacation, low-fiber diet are also common reasons for piles. Obesity, pelvic floor dysfunction, chronic cough prolonged sitting are some of the risk factors.

Types

There are two types of haemorrhoids; External and Internal Haemorrhoids

Signs & Symptoms

External haemorrhoids exhibit pain and swelling in the area of anus. If thrombosed it creates severe pain. If bleeding occurs it is usually darker. It is relieved after a few days and reccur again. A skin tag remains after healing of external haemorrhoids. It also causes irritation and itching around the anus. These are covered proximally by anoderm and distally by skin.

Internal haemorrhoids are often painless with bright red rectal bleeding while defaecating. They are painful only if  thrombosed or necrosed. If prolapsed produces itching and feacal incontinence. These are graded as

·         Grade I: No prolapse, just prominent blood vessels

·         Grade II: Prolapse upon bearing down, but spontaneous reduction

·         Grade III: Prolapse upon bearing down requiring manual reduction

·         Grade IV: Prolapse with inability to be manually reduced.

Pathophysiology

Anal canal is composed of haemorrhoidal plexus which supports in defaecation and anal closure on rest. When there is increased abdominal pressure the vascular structures engorge so as it slide downwards. Recurrent compression produces finger like projections protruding outwards while defaecation. External haemorrhoids are formed from inferior plexus and Internal heamorrhoids are formed from superior plexus.

Ayurvedic Piles Treatment

Ayurveda have a very successful conservative treatment for haemorrhoids which comprises of Intenal and External treatments along with diet regulations.

Ayurveda recommends preventive measures such as high fiber diets, drinking adequate water and yoga therapy. Strict vegetarian diet under doctor supervision so that constipation can be avoided. Ayurvedic internal medicine will correct the digestion process and thereby giving a good metabolic cycle. Yoga can also bring about changes in healthy digestive process and strengthen the pelvic floor.

Kshara treatment will pacify high grade piles devoid of severe inflammation. Avagahanam with disease specific decoction is another managing technique which pacifies the inflammation, itchiness and soreness.
